Responsibilities

• Coordinate patient care plans to ensure quality and continuity of care.

• Assess patient needs and develop individualized care plans.

• Monitor patient progress and coordinate appropriate healthcare services.

• Collaborate with physicians, nurses, and interdisciplinary healthcare teams.

• Assist with discharge planning and transitions of care.

• Advocate for patients and help ensure access to appropriate healthcare resources.

• Maintain accurate patient records and documentation.

• Ensure compliance with healthcare policies and procedures.

Qualifications

• Active Registered Nurse (RN) license.

• Minimum of 1 year of nursing experience.

• Previous case management experience preferred.

• Strong communication, organization, and care coordination skills.

• Ability to work independently and as part of a healthcare team.

• Must successfully pass a government background check.

• Ability to meet all facility credentialing requirements.
